Self-Evaluation Form 2017
While the complete evaluation packet isn’t due to the division office until Friday, December 21, the student surveys must be completed and back in the division office by Friday, October 20. Please start communicating early so that you can benefit from collegial collaboration and guidance.
Please find attached the current self-evaluation template (so that the evaluatee can type directly into the attached template for convenience). Notice that sections I and IV ask for a discussion of SLOs. The V.P.’s office spot checks for explicit and “robust” mention of SLOs in those areas.
In addition to addressing your department-level work on SLOs and assessments, you can also discuss how you teach and assess SLOs at the classroom level.
The evaluators, too, should include a discussion of SLOs in their observation reports and subsequent evaluation.
Recommendations for filling out the self-evaluation form
- OBJECTIVES FOR THE CONTINUED IMPROVEMENT OF INSTRUCTION AND COLLEGE COMMITTEE WORK/ACTIVITIES (E.G., STUDENT LEARNING OUTCOMES, ACADEMIC SENATE, PROGRAM REVIEW, CURRICULUM REVIEW/DEVELOPMENT AND/OR ANNUAL PLANNING):
I recommend focusing on SLO assessment at both the departmental and classroom levels. How do you make sure students are mastering the SLOs and objectives as they are listed on your syllabus? Regarding the other items, be thorough about all of your contributions at the department, division, and college levels.
- REFLECT ON ADJUSTMENTS YOU MAY HAVE MADE AS A RESULT OF STUDENT LEARNING/ACHIEVEMENT OUTCOME ASSESSMENTS:
Section II asks for a reflection on adjustments an instructor “may have made as a result of student learning/achievement outcome assessments.” In addition to overall curriculum changes (if any), this is a good opportunity to focus on how you might have made adjustments to your teaching during the semester as you assess your students’ learning and perhaps how you might make adjustments in the future based on what you’ve learned this semester through constant assessment of your students’ ability to achieve the SLOs. Let me know if you would like copies of the most recent SLO assessment reports for the courses you’re teaching this semester.
I also suggest that you do refer to student evaluations in section II (even though this revised template doesn’t mention them) as they can be a good source of information about the student experience from the student perspective. Because of this, you should complete your self-evaluation after you have received and read the student evaluations (though you may start it sooner).

I have attached the syllabus templates for English 1A and English 1C along with Jeff’s syllabi.
Please remember to include an attendance code key on at least one of the colored attendance rosters per class. You will turn those rosters in at the end of the semester, so please use them for record keeping. If you use another system (not recommended), you will still need to turn in these original rosters with whatever you’re using attached. We need a symbol for Present, Absent, and Tardy (please hold students responsible for attendance). Note that there are no “excused absences” at the college level (though the first day memos erroneously include a symbol for the same); the 10% rule applies.
